Information Collection, Use, and Sharing

Information You Provide to Us
In general, we collect and use personal information from you about yourself and your business for the purposes of responding to your requests for information from us about our services. The personal information we may collect may include your name, title, and contact information such as email address, telephone number, or physical address.

All such personal information is collected from you in the context of your role as owner or authorized manager of a business, healthcare practice, nonprofit agency, or other entity that might wish to do business with Cover Compliance. As such, the information collected is intended for a business-to-business commercial purpose and not for contacting you/marketing any services or products to you as an individual consumer.

We will use your information to respond to you, regarding the reason you contacted us. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, e.g. to ship an order. We will not sell or rent this information to anyone.

Unless you ask us not to, we may contact you via email in the future to tell you about new products or services, or changes to the privacy practices and conditions of use covered in this Notice.

Because the primary purpose of this Site is to describe the services Cover Compliance offers to commercial entities and to allow potential clients to contact us, we do not knowingly receive any information about individuals who are customers or patients of commercial entities with whom we communicate through this site. We discourage anyone who provides us with their own contact information through this Site from providing ANY personally identifying information or protected health information about any individual other than providing their own information related to their contact request. We encourage information requestors visiting this Site to only provide the specific data elements needed to honor the contact request.

Because of the nature of our business, our services are not designed to appeal to minors. We do not knowingly attempt to solicit or receive any information from children.

Our services are intended for businesses and organizations in the United States. We do not solicit business from non-U.S. entities through this Site and would only inadvertently collect contact information through this Site from a non-U.S. person.
URL and IP Address
Like many other web sites, we collect information about our users' utilization and navigation of the Site. This information helps us to design our sites to better suit our users' needs. We may also use your IP address to help diagnose problems with our server and to administer the Site, analyze trends, track visitor movements within the Site, and gather broad demographic information that assists us in identifying visitor preferences.

Cookies and Web Beacons
Cookies are pieces of data in the form of small text files that a web site transfers to a user's computer for record-keeping purposes. Web beacons are transparent pixel images that are used in collecting information about web site usage, email response and tracking.

The Site uses cookies and web beacons to provide enhanced functionality on the Site (e.g., customized responses to information request forms) and aggregate traffic data (e.g., what pages are the most popular). These cookies may be delivered in a first-party or third-party context. We may also use cookies and web beacons in association with emails delivered by Cover Compliance.

Our site also captures limited information (e.g., web browser information including user-agent, HTTP referrer, last URL requested by the user, client-side and server-side clickstream) about visits to the Site. We may use this information to analyze general traffic patterns and to perform routine system maintenance.

In addition to our own use of cookies and web beacons in Cover Compliance’s web hosting functionality, this Site uses Google Analytics, a web analytics service provided by Google, Inc. ("Google"). Google Analytics also uses cookies to help the Site analyze how users use the Site. The information generated by the cookies about your use of the Site (including your IP address) will be transmitted to and stored by Google on servers in the United States. Google will use this information for the purpose of evaluating your use of the Site, compiling reports on site activity for web site operators and providing other services relating to web site activity and internet usage. Google may also transfer this information to third parties where required to do so by law, or where such third parties process the information on Google's behalf. Google will not associate your IP address with any other data held by Google.

You have many choices with regards to the management of cookies on your computer. All major browsers allow you to block or delete cookies from your system. To learn more about your ability to manage cookies and web beacons, please consult the privacy features in your web browser(s). You may refuse the use of cookies by selecting the appropriate settings on your browser; however, please note that if you do this you may not be able to use the full functionality of this Site.

Interfaces with Social Media Sites
As this Site may contain links to other web sites, please be aware that we are not responsible for the privacy practices of such other sites, and we are not liable for their misuse of personally identifiable information. We do not provide any personal information to other sites through any links this Site might contain. We encourage you to be aware that when you go to another web site you should carefully read their privacy notices and terms of use for that site.

This Site uses interfaces with social media sites such as Facebook, LinkedIn, Twitter, and others to facilitate our visitors’ engagement with their own social media networks regarding their reactions to information on this Site and their engagement with Cover Compliance based on their use of this Site. If you choose to "like" or share information from this Site through such services, whether by direct posting within one of those social media sites using a hyperlink to this Site or any page or blog post within it or by using the social media site icons placed on this Site, you should review the privacy policy of that service. If you are a member of a social media site and you choose to “like” or share information from this Site, the interface with that social media site may allow that site to connect your site visit on this Site to your personally identifiable information accessible on that social media site or accessible to that social media site by other means independent of this Site’s functionality.

Your Access To and Control Over Information

You may opt out of any future contacts from us at any time by contacting us at the email address or phone number at the bottom of this Notice or by submitting your request through the contact form on this Site. You can do the following at any time by contacting us:
  • See what data we have about you, if any.

  • Express any concern you have about our use of your data.

Security

We take precautions to protect your information. When you submit sensitive information via the Site, your information is protected both online and offline.

Wherever we collect sensitive information (such as your personal contact information), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for "https" at the beginning of the address of the web page on this Site where such information is requested. Currently our contact form is the only place on this Site where such collection exists.

Please note that email is not normally considered a secure method of communication unless you maintain a “secure email” service that uses encrypted transmission of your messages. Since email is not normally considered secure, we discourage sharing sensitive information through email, including through attachments unless the attachments are encrypted and the means of accessing the data (i.e., the password for the attachment) is provided to us through a separate communication channel. In addition, any time such information is going to be sent via email, we advise others to communicate with us directly by phone to get an advance notice of the information to be provided and the means of transmission of the information in a communication that is separate from the transmission of the information itself.

While we use encryption to protect sensitive information transmitted online, we also protect your information when the date is stored at rest and offline. Only individuals who need the information to perform a specific job (for example, billing or customer service responses) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are encrypted and physically secured.
